Chronic Low-Level Lead Exposure and Neurodevelopmental Outcomes in Children: A Narrative Review of Physiological Mechanisms and Public Health Implications

Chronic low-level lead (Pb) exposure remains a significant environmental health concern and one of the leading preventable causes of neurodevelopmental impairment in children worldwide. Accumulating evidence indicates that adverse cognitive, behavioral, and motor outcomes occur even at blood lead concentrations previously considered safe, with no identifiable threshold below which adverse effects are absent. This narrative review synthesizes evidence from 25 landmark prospective cohort studies, longitudinal follow-up investigations, and meta-analyses to evaluate the neurodevelopmental consequences of chronic low-level lead exposure, examine the physiological mechanisms underlying these effects, and discuss their public health implications. Studies meeting a priori inclusion criteria—including prospective or longitudinal design, child populations, quantified low-level lead exposure, validated neurodevelopmental outcomes, and appropriate adjustment for confounding variables—were reviewed and synthesized thematically. The evidence consistently demonstrates an inverse association between blood lead concentration and intellectual performance, with several studies reporting disproportionately greater reductions in intelligence quotient (IQ) at the lowest exposure levels. Chronic exposure has also been associated with attention-deficit/hyperactivity disorder (ADHD), behavioral and emotional disturbances, juvenile delinquency, impaired motor development, and structural brain abnormalities extending into adulthood. Proposed physiological mechanisms include calcium mimicry, disruption of calcium-dependent neuronal signaling, oxidative stress, mitochondrial dysfunction, altered neurotransmitter release, impaired synaptic plasticity, neuroinflammation, and epigenetic modifications that interfere with normal brain development. Collectively, the reviewed evidence confirms that chronic low-level lead exposure produces measurable and irreversible neurodevelopmental deficits, emphasizing that no safe level of childhood lead exposure has been established. These findings reinforce the need for strengthened environmental surveillance, early childhood screening, effective exposure prevention strategies, and evidence-based public health policies aimed at eliminating preventable lead exposure and promoting environmental health equity.
